Rebuilt to get new version number

Remove MKS dependency now that Cygwin cat works on binary files
without munging with carriage returns and newlines.

Added #ifndef __CYGWIN__ around some pesky extern references.

shivers 2baee835a2 Tty-hacking procs now default to stdio when a tty isn't specified. 
You may now apply them to a file-name as well as a port or fdes.
